Community Health Workers/Promotores

Community Health Worker/Promotores (CHW/P) goals are to promote health equity and access and address social determinants of health. CHW/P do this by connecting patients and community members to health care access and wellbeing services and collaborating with community-based organizations with health education presentations and outreach.

Our CHW/Ps provide individuals and their families’ access to primary and preventive care by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community/patient education, community resource connections and advocacy.
